We are committed to the highest professional standards in dispensing your prescriptions and counselling you about medications.


We also keep records of your prescriptions and other relevant health information.

As well, we may need to speak to your doctor, other health care providers, and other pharmacies in order to obtain the information to enable us to provide pharmacy care to you.

 


Your Personal Information, Your Privacy, Your Consent - Why?

On January 1, 2004, federal privacy legislation (PIPEDA*) came into effect, and requires pharmacists and pharmacy staff to obtain your formal consent to collect, use and disclose your personal health information.

This information may include, but not be limited to, your name, address, telephone number, and information regarding your prescriptions. We will now need your formal consent to collect, use and disclose your personal information.

Some or all of the information may be information that we currently have on record.

If you have brought us a prescription, medication order, or request for pharmacy services, you have consented to the collection and use of your, your children's, or other dependent's personal information, including the sharing of such information with your insurance and health care providers.
